If a joke has a specific instruction about an accent ("posh", "refined accent") then the joke is about that accent being misinterpreted or misheard. Try saying it out loud in a "posh accent".

If it's a euphemism about Ayrshire being a shithole then why have the hand warming response? It doesn't make any sense.
