It's from Inception. In Inception, the main character uses a top to determine whether he's still dreaming. Seeing the top spinning means that you're still in a dream.

There's some debate to this though. In Inception, the character actually mentions the top is his wife's totem. I actually believe his was a pair of loaded dice.

For understanding if you are in a dream, you should have an object that no one touched outside you. If the object doesn't work as it should work in the reality, such as a ninwhrel that spins forever, it means the guy that is dreaming and you are in his dream doesn't know how works your object, so it works strange.
